Colonial Acres is a Five Star Skilled Nursing Home and Assisted Living located in Humboldt, Nebraska. We are community-owned and we pride ourselves on providing first class resident care while maintaining a friendly, family atmosphere. Our beautiful facility includes an enclosed courtyard with waterfall and lovely landscaped patio areas.

Colonial Acres is located at 1043 10th Street, Humboldt, Nebraska—approximately 100 miles south of Omaha and 75 miles southeast of Lincoln.

We have very strong QAPI protocols and we have had no rehospitalizations of Medicare patients in over two and one-half years. We are very proud of our 5-Star status and we work diligently to maintain it.

Colonial Acres’ Medical Director, Dr. George Voigtlander, works with CMS on clinical pharmaceutical standards among the elderly. We are currently participating in a clinical trial of a new influenza vaccine for seniors and we are one of a handful of facilities in the State of Nebraska to participate in a Center for Disease Control (CDC) study on antibiotic stewardship and C-Diff/MDRO prevention. We cooperate closely with VA clinics and VA pharmacies to provide care for our veterans.

Primary Care Clinician Services:

Weekly on-site physician, NP or PA rounds from Pawnee City Rural Health Clinic and site visits by Falls City physicians.

Comfortable transportation for residents with physicians in Auburn, Falls City, Pawnee City,  Tecumseh and surrounding areas.

Transportation to specialty clinics in the local area as well as to appointments with specialists in Lincoln,   Omaha, Beatrice, Falls City and Tecumseh.
